Advising Agreement Sheet
In order for J.D.M Consulting to proceed with the services it provides, it is necessary that all parties involved, fully understand the following points:

1. J.D.M. Consulting agrees in accordance with NCAA rules, to provide the Program requested and agreed upon by the parties involved.

2. The fee associated with J.D.M. Consulting full Program package is a flat fee. There are no contingent fees if a scholarship or financial aid package is awarded.

3. The student athlete agrees to indemnify and hold harmless J.D.M. Consulting and any of its agents and officers.

4. J.D.M. Consulting makes no guarantees or promises that a scholarship or financial aid package will be offered by any college or university to which J.D.M. Consulting services are extended.

5. The student athlete agrees to submit to J.D.M. Consulting, a report of any communication with a college or university within two weeks of contact.

6. J.D.M. Consulting will maintain a file on each student athlete in order that the information be provided to various colleges and universities. The file will be maintained and updated. Any changes of address or teams should be forwarded as soon as possible.

7. Once a client has retained JDM services no refunds or pro-rated refunds will be available.
